To print the application, click on the pages below one at a time. When the page is displayed in Internet Explorer, click on File then click on Print Preview and when the print screen is displayed, in the top menu bar, make sure the box is set to "Shrink to Fit", and print the document.  When you have finished printing a page, click the link at the bottom of the page to go back to the menu or continue to the next page.  Repeat the process until all 4 pages are printed.  Fill out the application and fax it to 325-738-8529 or you can mail it in or you can bring it in personally.
